Code G and the 1040

Code G is a direct rollover to an IRA. You need to answer all the follow up questions before you can file your tax return. Normally you wouldn't receive a check with a direct rollover, but the funds would be sent to the financial institution where the IRA is.

If there are multiple beneficiaries, report the amount paid to each of them on Form 609-R, and enter the percentage in box 9a. You must give a statement to the recipient if you are required to file Form 1099-R.
